Forms & Documents (E-Signatures)

Build hosted forms and legally binding e-signature documents.

  • What's the difference between Forms and Documents?

Forms are public-facing intake pages — surveys, registrations, lead capture, consent collection. They're hosted on Respectly, mobile-optimized, and can be embedded or distributed via a link, QR code, or message. Documents are e-signature workflows — agreements, contracts, consent forms — that one or more specific signers complete in a specific order, producing an audit trail and a signed PDF.

Use Forms for "anyone who clicks this can submit" and Documents for "this specific person needs to sign this specific agreement."

  • How do I create a form?

Open Forms → New and either use the drag-and-drop builder or describe what you want in the AI prompt box ("a contact form with name, email, phone, and a consent checkbox") and let the AI assemble it. Field types include text, email, phone, address, date, dropdown, checkbox, file upload, signature pad, and rating. Forms are auto-branded with your logo and colors.

  • Can I host forms on my own domain?

Yes, on Pro and Enterprise only. Set up a CNAME record pointing to Respectly and configure the custom domain in Settings. Forms will then be served from forms.yourdomain.com (or any subdomain you choose) instead of the default Respectly URL.

  • Are forms PIN-protected?

PIN protection — requiring a short numeric code before the form opens — is available starting on Standard. Use it for sensitive intake forms where you want to make sure only the intended recipient can submit.

  • Do you support legally binding e-signatures?

Yes. Respectly's e-signatures comply with the U.S. ESIGN Act and UETA and equivalent frameworks in other supported countries. Each signed document includes a complete audit trail: signer identity, IP address, timestamps for view/sign events, and a tamper-evident sealed PDF. Multi-party signatures support sequential workflows where each signer is notified in order.

  • What happens after a form is submitted?

Submissions create or update a contact, store the field values, fire any associated workflows, and notify the configured team members. If the form captured consent, the consent record is created with full evidence (timestamp, IP, disclosure text shown, signature/checkbox state). All submissions are exportable to CSV.

  • Are there limits on form submissions?

Free is capped at 25 submissions/month, Standard at 200/month, and Pro and Enterprise are unlimited. Submissions beyond the cap on lower tiers are blocked until the next billing cycle or until you upgrade.
